The movie follows three high school friends, Samantha, David and Amy, who attend their prom together. During the prom, the trio witness a student receiving a strange mark on her hand. Later that night, they hear about the legend of Bloody Mary and decide to perform the ritual to summon her in front of a mirror.
After performing the ritual, strange and terrifying things start happening to them, including manifestations of a vengeful spirit who appears to be Bloody Mary herself. Samantha becomes the primary target of the spirit's wrath, as her family has a dark history related to the legend.
As the spirit becomes more dangerous and violent, the three friends try to find a way to stop it from causing any more harm. They eventually uncover the truth behind the legend and the reasons for the spirit's anger. In the end, they are able to put an end to the curse and free themselves from the deadly consequences of unleashing Bloody Mary.
