In Silent Night, Deadly Night III: Better Watch Out!, Ricky Caldwell has been in a coma for six years after being shot in the head at the end of the second movie. During this time, he has been kept alive on a life-support machine, which is powered by a psychic doctor named Newberry. Newberry believes that Ricky's condition will allow him to tap into what he calls the "psychic plane," which he believes will give him superhuman powers.
When Ricky awakens from his coma, he is missing an eye and has a robotic hand. He is also mute, although he can communicate telepathically with Newberry and can occasionally speak using a speech synthesizer.
Ricky immediately sets his sights on Laura, a clairvoyant blind woman who is traveling with her brother Chris and his girlfriend Jerri to their grandmother's house for Christmas Eve. Laura's psychic abilities allow her to see glimpses of the future, and Ricky sees her as a threat to his plans.
As Chris, Jerri, and Laura make their way to their grandmother's house, Ricky follows them, leaving a trail of dead bodies in his wake. He is aided in his quest by a group of punks who worship him as their leader, although they are unaware of his true identity.
