Saw IV begins with the autopsy of the infamous Jigsaw killer, John Kramer (Tobin Bell), revealing that he died from cancer and did not die from his wounds during his capture in Saw III. Detective Mark Hoffman (Costas Mandylor) is revealed to be Jigsaw's secret apprentice, and he continues to carry out Jigsaw's legacy by creating traps for new victims.
Meanwhile, SWAT team leader Eric Matthews (Donnie Wahlberg), who was captured by Jigsaw in the previous film, is still missing and presumed dead. However, when Detective Rigg (Lyriq Bent) is abducted and wakes up in a trap, he is given 90 minutes to save Matthews before he is killed.
As Rigg moves through different rooms, he encounters various people connected to Jigsaw's past, including Detective Hoffman and FBI profiler Agent Strahm (Scott Patterson). The traps become increasingly gruesome and twisted, and Rigg must make difficult choices to try and save Matthews.
As the game unfolds, Agent Strahm begins to suspect Hoffman's involvement with Jigsaw and starts investigating him. This leads to a dangerous cat-and-mouse game between the two, with both men trying to outsmart each other in a deadly game of wits.
In the end, Rigg fails to save Matthews and is killed in a trap, while Strahm uncovers the truth about Hoffman's involvement with Jigsaw and is trapped in a final twist ending. Saw IV lays the groundwork for a new chapter in the franchise, setting up the events of Saw V and VI.
