The scuba diving instructor, Anne Kimbrough, and her biochemist boyfriend, Tyler Sherman, are called to investigate a series of strange deaths occurring near a Caribbean island resort where Anne works. The victims have been found with missing limbs, and their bodies are covered in strange bite marks. Anne immediately suspects that piranha fish are responsible for the deaths.
However, the police chief ex-husband, Steve, is skeptical of Anne's theory and dismisses it as a silly conspiracy. Despite Steve's reluctance to believe Anne's theory, the evidence quickly builds up, and it becomes clear that the deaths are indeed being caused by a mutant strain of piranha fish.
Anne and Tyler team up to try and stop the piranhas, but they soon discover that the fish have been bred in a sunken freighter ship and have been feeding on toxic waste, which has made them nearly invincible. As the death toll continues to rise, Anne and Tyler must find a way to destroy the sunken ship and eliminate the mutant piranhas before they can cause any more damage.
In a thrilling climax, Anne and Tyler set explosives on the sunken ship where the piranhas are holed up, sending the ship to a watery grave along with the deadly fish. As the credits roll, the audience is left wondering if any of these monsters could have survived or if only water took them away.
