After the car crash, Paul Sheldon is rescued by Annie Wilkes, a former nurse who lives in a remote cabin. Annie is a devoted fan of Paul's books, particularly his series featuring the character Misery Chastaine. Paul is badly injured and unable to move, and Annie takes him in and cares for him.
As Paul begins to recover, he realizes that Annie is not as innocent as she seems. She has a dark side and is prone to violent outbursts when she is displeased with Paul's writing. Annie also reveals that she has been keeping Paul isolated from the rest of the world and has been reading his latest manuscript, in which he plans to kill off the character of Misery.
Annie becomes increasingly unstable and obsessed with Paul's work, forcing him to rewrite the manuscript to save Misery's life. She also begins to subject Paul to physical and psychological torture, keeping him heavily medicated and confined to his bed. Paul tries to find a way to escape, but Annie has cut off all communication and locked him in.
Eventually, Paul realizes that his only hope of survival is to play along with Annie's delusions and convince her that he loves her and that Misery is still alive. He manages to manipulate her into letting him out of his room and attempting to leave the house, but Annie catches him and becomes even more enraged.
In the end, Paul is able to overpower Annie and kill her, escaping from the cabin and seeking help. He is rescued by the police and taken to a hospital, where he is finally able to recover from his injuries and begin a new life.
