Pinhead, the leader of the Cenobites, finds himself trapped inside a sculpture called The Pillar of Souls, that was created by a deranged artist. The sculpture is sold to J.P. Monroe, a rich playboy and owner of a popular night club, who displays it in the club's lobby. As the sculpture's power grows, Pinhead uses J.P.'s hedonistic lifestyle to his advantage, and lures unsuspecting victims to their doom.
With each sacrifice, Pinhead's powers increase and he starts manifesting in the real world. He is determined to destroy the puzzle box known as the Lament Configuration, which is the key to his eternal imprisonment in Hell. His plan is to lure a human host to solve the puzzle box for him, in exchange for their desires. That's when he meets Terri, a young television reporter who is investigating the murders linked to J.P. Monroe and his night club.
As Pinhead and his minions begin wreaking havoc on the town, Terri realizes the true nature of the evil that she's facing. With the help of a young bartender named Joey, she sets out to stop Pinhead and close the portal leading to Hell. But Pinhead is relentless, and with each victim he claims, he grows stronger. Terri must summon all her wits and courage to face the ultimate terror and stop Pinhead before it's too late.
