The movie begins with Michael Myers being shot multiple times and falling into a mine shaft, presumed dead by the police. However, a mysterious man dressed in black rescues Michael and secretly nurses him back to health. A year later, on Halloween night, Michael returns to Haddonfield and begins his killing spree once again.
Meanwhile, Jamie Lloyd, Michael's young niece who he has been obsessed with since the previous film, is now in the care of a foster family. She has been traumatized by her experiences and has lost her ability to speak. Jamie has a psychic link with her uncle, allowing her to see and feel his actions.
As Michael targets Jamie's new family and friends, she becomes increasingly disturbed by her psychic connection with him. Dr. Loomis, Michael's longtime psychiatrist, returns to Haddonfield to help stop him once and for all. He discovers that the mysterious man in black seems to be guiding Michael's actions and may hold the key to stopping him.
The film ends with a twist, revealing that the man in black is actually a member of the Myers family and has been manipulating Michael to achieve his own goals. Jamie is left traumatized but alive, and Michael's killing spree is finally put to an end.
