The protagonist of the movie is named Anna Marchant (played by Milla Jovovich), a young woman who survives an encounter with a serial killer, whom the media dubs "The Tearjerk Killer". However, during the attack, Anna sustains a head injury that results in her developing face-blindness, or prosopagnosia.

Anna struggles to cope with her condition, which makes it impossible for her to recognize people by their faces. Each time she sees someone, their face may look completely different than before, making it difficult for her to navigate through daily life.

As the story unfolds, Anna begins to suspect that the Tearjerk Killer is still on the loose and is closing in on her. Despite her inability to recognize his face, she begins to experience vivid flashbacks of their initial encounter.

With the help of a neurologist, Dr. Langenkamp (played by Marianne Faithfull), Anna attempts to regain her memory and find a way to identify the killer before he strikes again.
