In Crank: High Voltage, Jason Statham reprises his role as Chev Chelios, a hitman who survived falling from a helicopter in the first movie, but is now being pursued by a Chinese gangster named Johnny Vang (played by Art Hsu). After being kidnapped and taken to a secret location, Chelios wakes up to find that his heart has been surgically removed and replaced with a battery-operated artificial heart. He is given a shock collar that will provide the electricity needed to keep his heart pumping, but the battery only has a short lifespan.

Chelios sets out on a frantic mission to find his real heart, which has been stolen and put up for sale on the black market. Along the way, he encounters a variety of colorful characters, including a stripper with a heart condition played by Bai Ling and a talkative cabbie played by David Carradine. He also has to contend with Vang and his henchmen, who are determined to capture him and retrieve the heart.

As the clock ticks down on his borrowed heart, Chelios uses every means at his disposal to stay alive, including drugs, violence, and even electrocution. The action is fast-paced and over-the-top, with plenty of explosions, car chases, and hand-to-hand combat. In the end, Chelios manages to retrieve his stolen heart and reconnect it to his body, but not before unleashing a final showdown with Vang that leaves both men battered and bloodied.
