In Citizen Toxie: The Toxic Avenger IV, the Tromaville School for the Very Special is taken hostage by a group of disgruntled former students known as the Diaper Mafia. The school is known for its inclusive and diverse student body, which includes those with disabilities and special needs.
To save the students and faculty from certain doom, the Toxic Avenger teams up with his trusty sidekick Lardass, a morbidly obese man who has the ability to vomit on command. Together, they set out to take down the Diaper Mafia and save Tromaville from their evil grasp.
As they journey through the school, they encounter a wide array of strange and eccentric characters, including a blind student with deadly ninja skills, a sexually ambiguous janitor, and a group of mentally challenged students who have taken on the personalities of famous movie characters.
Along the way, the Toxic Avenger and Lardass face a series of obstacles and challenges, including a deadly game of dodgeball against the Diaper Mafia and a final showdown with the group's leader, a former student who has become a powerful and dangerous villain.
