The four friends, Brian (Chris Pine), his girlfriend Bobby (Piper Perabo), his brother Danny (Lou Taylor Pucci), and Danny's friend Kate (Emily VanCamp), have a strict set of rules to avoid infection: avoid contact with anyone outside of their group, always wear protective gear, and kill anyone who shows symptoms.
As they travel, they come across a father (Christopher Meloni) and his infected daughter (Kiernan Shipka). The group debates whether to help them and risk infection or leave them to die. Brian, the natural leader, decides to leave them on the side of the road.
Their journey becomes more perilous as they encounter others who are infected or desperate to avoid infection, including a sickly old man (Frank Bond) and a cult-like group led by a charismatic but unhinged man (Mark Moses).
After their car breaks down, the group seeks refuge in a deserted motel. Danny, who has been hiding his own infection, becomes increasingly ill, and tensions rise as the group debates what to do.
In the end, the group's strict adherence to their rules causes them to make difficult but ultimately fatal decisions. In this grim world, survival is the ultimate priority, but at what cost?
